Primary Care Registered Nurse Position Details

  • Status: Full-time, 12-month contract with the possibility of extension
  • Company: South Calgary Primary Care Network
  • Schedule: Monday – Friday, Standard 38.75 hour work week
  • Location: PCBH providers work within South Calgary family doctors’ offices
  • Reports to: Program Manager, Primary Care Services

Position Summary

The Primary Care Registered Nurse (PCBH Provider) will collaborate with the medical home team to establish a comprehensive support and service system within the patient’s medical home. The role involves providing population-based, high-quality healthcare grounded in empirical evidence to patients, families, and communities.

What Does the Role Do?

  • Identify, triage, assess, and provide behavioral strategies and support to primary care patients with mental and physical health concerns.
  • Receive extensive training in the PCBH model to employ evidence-based treatment interventions and best practices.
  • Deliver care to families, groups, and patients of all ages, stages, and abilities.
  • Collaborate, educate, and consult with internal and external healthcare teams, referral organizations, and community organizations.

Who Should Apply?

  • Social work, Registered Psychiatric Nurse, or Bachelor of Science in Nursing.
  • Enrollment in a college authorized by the Health Professions Act.
  • Current BLS (Basic Life Support) certification for medical professionals.
  • Minimum of two to three years of experience in psychiatric emergency roles and/or community-based settings as a registered nurse or social worker.
  • Prior experience in a primary care setting or within a medical home/integrated care model is preferred.
  • Familiarity with electronic medical records is advantageous.
  • Experience in group development, facilitation, or instruction is a strong advantage.
  • Training and experience in primary care behavioral health models, health-related behavior change methods, and/or cognitive behavioral therapy are assets.
